Job Description

1. Performs all required responsibilities/duties addressing the ages and condition of the patients served. Responsible for direct patient care which is determined, observed, supervised and documented on plan of care by a professional clinical provider.


2. Performs delegated tasks as authorized by the professional clinical provider depending upon situation and HHA/HCA III’s experience/competence. Demonstrates the ability to perform selected activities once approved and as observed and supervised by the clinical provider.


3. Performs routine household tasks as related for a safe, healthy and comfortable environment for the patient, as instructed and documented on plan of care by the professional clinical provider.


4. Documents patient status and care given, as reflected on required records. Submits documentation in a timely manner according to Agency guidelines/deadlines.


5. Works as part of the Home Health Care Team participating in and identifying ways to improve the quality of care being provided to the patient. On-going attendance at in-service programs, minimum 12 hours per calendar year, to maintain certification.


6. Safety Awareness – Fosters a “Culture of Safety” through personal ownership and commitment to a safe environment. Complies with current hand hygiene and infection control guidelines.


7. Telemonitoring- Works with the Telehealth manager and HHA scheduler to assist with installation and removal of telemonitoring equipment in the patients home. Instructs patient/family in proper use of telemonitoring equipment.


Good communication skills, which includes ability to read, write, speak and understand the English language effectively. Demonstrates satisfactory compliance with State and Federal Requirements for Home Health Aide/HCA III competency Must demonstrate maturity and ability to deal effectively with issues related to delivery of services. Must be able to drive/or use public transportation to patients’ homes.
