REHAB TEAM LEADER

Job Description Summary

Under the supervision of the Clinical Nurse Manager, the Team Leader will be involved in daily operations of the team, including staff education, supervisory visits, special projects as well as acting Clinical Nurse Manager when the Clinical Nurse Manager is out sick or on vacation. The Team Leader will work in collaboration with the Clinical Nurse Manager on productivity and HHRG management as well as Quality of Care issues identified by PIE. Team Leaders will also continue to make visits in the community based on time and need. The Team Leader will assist in the promotion of quality of care in the field as well as documentation in the medical record. The Team Leader will be a resource to all members of the team.

Job Description

ESSENTIAL FUNCTIONS

1 - Documentation

   a - Demonstrate complete, clear, concise documentation in the medical record

   b - Timely documentation in the medical record: according to the guidelines established by the agency.

   c - Ability to communicate and demonstrate documentation requirements to another staff person.

   d - Monitor discharge documentation and communicate areas for improvement.

   e - Monitors HHABN distribution on all patients.

2 - Leadership

   a - Ability to listen to staff and reframe their needs.

   b - Ability to be accountable to the role as Team Leader.

   c - Ability to identify and escalate issue to the appropriate person. 

   d - Ability to support staff as needed.

   e - Ability to be responsible for the Rehab Manager role when needed.

   f - Attend appropriate meetings as Rehab Manager representative when needed,

   g - Develop team cohesiveness with introduction and support of new team members.

   h - Ability to be flexible to the needs of the team and the agency.

   i - Ability to work with other teams and departments within the agency to deliver the best outcome.

   j - Attends meetings as requested.

3 - Special Projects

   a - Ability to identify project objective and expected outcome and communicate them.

   b - Ability to work unsupervised to deliver project within the specified deadline.

   c - Ability to identify/suggest Special Project that would benefit the team overall.

   d - Ability to communicate special project, expected outcome and next steps.

   e - Ability to seek guidance when needed.

   f - Ability to identify the need to work with other teams and departments within the agency to deliver the best outcome.

4 - Supervision

   a - Make supervisory visits with staff in the field identifying and communicating areas for improvement.

   b - Make supervisory visits with staff in the field identifying and communicating areas of expertise and communicating to the Rehab Manager.

   c - Follow up with any staff person who may be having areas of difficulty, communicating timelines and issues to Clinical Nurse Manager.

   d - Act as power of example for staff to complete work timely and within the agency's guidelines.

   e - In collaboration with Clinical Nurse Manager identify high risk patients to be discussed in a team meeting

5 - Support and Encouragement of staff

   a - Identify staff that would be candidate for recognition, awards etc.

   b - Assess on going needs of staff and communicate to the leadership, as needed.

   c - Continually assess need for ways to show appreciation to staff in collaboration with Rehab Manager.

   d - Ability to support staff from other teams and the agency to provide the best outcome.

6 - Communication

   a - Communicate with Clinical Nurse Manager according to expectations.

   b - Communicate with leadership as needed and as Clinical Nurse Manager representative.

   c - Communicate with staff on -going.

   d - Continually assess your need for communication improvement and seek appropriate course.

   e - Communicates with other teams and within the agency to provide the best outcome.

7 - Visits

   a - Continue to carry a case load dependent on the need of the team and the agency.

   b - Work with Clinical Nurse Manager to identify the size of your caseload and functions in the office on a monthly basis.

JOB REQUIREMENTS

Minimum Education - Preferred

Bachelor's of Science

Minimum Work Experience

2-3 years experience preferred

Required Licenses / Registrations

PT or OT - Physical Therapist OR Occupational Therapist

Required additional Knowledge, and Abilities

OASIS - C, Allscripts, Documentation requirements, Discharge documentation and HHABN. Work with the Clinical Nurse Manager to complete Reports and items on thework ques in a timely manner
