Job Description Summary

The infusion community health nurse is a professional nurse knowledgeable in infusion nursing who provides specialized care to patients in the home.

Job Description

1 - Makes scheduled home visits to patients as evidenced by documentation in the clinical record and nursing daily log. Schedules and prioritizes nursing visits according to client acuity and in accordance with the organization's emergency preparedness plan

2 - ASSESSMENT/ADMISSION
During admission visit of the patient infusion, assesses and identifies patient and family needs in order to provide specialized care to assigned patients, as evidenced by documentation in the clinical record within twenty–four hours of admission. Performs admission physical and psychosocial assessment to determine nursing needs.

3 - For each admission, develops an individualized and holistic Plan of Care for the patient with infusion needs in collaboration with the patient/family/caregiver, and infusion vendor as evidenced by documentation in the clinical record. Updates the plan of care on an ongoing basis as goals are met and problems are identified.
 

4 - REASSESSMENT/VISIT DOCUMENTATION
During each visit the Plan of Care, Teaching Plan, Discharge Plan and outcome criteria are reassessed to meet patient and family/caregiver needs and assure quality of care, as evidenced by documentation in the clinical record.

5 - DISCHARGE/DOCUMENTATION DISCHARGE
For each discharge, documentation demonstrates improvement in outcomes or applicable variances are identified. Patients are appropriately prepared for discharge and all disciplines involved in the care of the patient are in agreement with plan. Vendors and all MD offices and clinics involved in the care of the infusion patient are notified.

6 - MEDICATION

Demonstrates knowledge of department policies and procedures for administering and documenting medications. Medication reconciliation is completed every visit and medication profile is reviewed and updated with changes as appropriate. DRR is run with each additional medication. Medication management is taught to each patient/family member each visit, including side effects.

 

7 - CASEMANAGEMENT

The case manager demonstrates responsibility and accountability for coordinating the plan of care for each patient on caseload. The patient /family is involved and participates in the plan of care. Communicates with all disciplines to insure quality of care, including vendors/pharmacists/ID clinics.

 

8 - SAFETY AWARENESS

 Fosters a “Culture of Safety” through personal ownership and commitment to a safe environment. Complies with current hand hygiene and infection control guidelines.

9 - PROFESSIONAL DEVELOPMENT

Responds to and participates in opportunities to maintain, enhance, and promote the image of the professional nurse. Maintains CRNI certification if applicable. Attends Infusion seminars and learning sessions.

11 - SAFETY AWARENESS
Fosters a “Culture of Safety” through personal ownership and commitment to a safe environment.
