Under the general guidance of the clinical director and in accordance with 244CMR 4.00 of the Massachusetts Board of Registration in Nursing, the Cardiovascular Nurse Practitioner will evaluate, diagnose and treat patients with Cardiovascular Disease and associated health conditions following written guidelines approved by the Department of Medicine.

1. Performs evaluations, including: pertinent history and physical exam on patients assigned to the Hospitalist Team. Institutes a plan of care based on the findings. Informs the responsible physician of pertinent subjective and objective data and routine diagnostic and therapeutic measures initiatied.
a. Documents clinical findings according to the SSH documentation policies and procedures

b. Consults physician when necessary, prior to proceeding with care.

c. Orders diagnostic tests and follow up on results.

d. Prescribes required medications and treatments.

e. Coordinates subspecialty referrals in consultation with the responsible physician.

f. Provides education and follow up instructions for the patient and family.


2. Performs daily rounds on cardiac patients assigned to the hospitalist team.
a. Performs history and physical exam, formulates a plan of care and documents findings after a discussion with the hospitalist

b. Works with the hospitalist to expedite the interpretation of diagnostic tests to facilitate medical managment and decision making.

c. Serves as a liaison between the cardiologist and hospitalist services to ehance communication and patient flow.

d. Collaborates with Nursing, Case Management and other members of the multidisciplinary team on a daily basis.

e. Actively participates in daily multidiscipliary rounds on the cardiovascular unit.


3. Facilitates the Discharge of Cardiac Patients
a. Performs discharge history and exam, completes discharge documentation and dictates the discharge summary after communication with the hospitalist.

b. Facilitates and coordinates adjunctive services upon discharge in collaboration with Nursing and Case Management. (Home care, cardiac rehab, etc)

c. Facilitates transfers of patients on and off of the cardiac unit.

d. Facilitates transfers of patients to tertiary hospitals as needed.

e. Provides education for the patient and the family related to post discharge care, medications and follow up.


4. Serves as a clinical resource to the multidisciplinary team
a. In collaboration with the Cardiovascular Nurse Manager and Clinical Nurse Specialist, assists in the formal education of the nursing staff.

b. Participates in the quality monitoring and improvement activities.

c. Assists in the development of treatment procedures and protocols.


5. Works with Case Management, Nursing Leadership and the Hospitalists to improve the flow of patients.
a. Collaborates with the team to predict discharges, coordinate testing and complete the discharge process in a timely manner

b. Identifies areas for system improvement to help faciliate discharge.

6. Evaluates the use of the cardiovasular resources for appropriateness
a. Collaobrates with cardiovascular nursing leadership to determine the appropriateness of the use of the expert cardiac nurse resources on P5

b. Coordinates use of the cardiovascular resources in conjunction with the Hospitalist and Cardiologists.

c. Makes recommendations to prevent inappropriate admissions from coming to P5 and daily identifies patients on P5 who may be transferred to a lower level of care.

d. Collaborates with the Patient Flow Coordinator on patient placement.

e. Ensures proper criteria are met for admission, diagnostic testing and transfer of cardiac patients.


Minimum Education - Preferred
Master of Science in Nursing from a NLN accredited graduate program required.
Minimum Work Experience
Two years of clincial practice as a Nurse Practioner including experience with the care of the Cardiovascular Patient

Demonstrated history of proficiencyin the delivery of medical care to adult patients.
Requires ACLS certification as tracked by the Medical Staff Office
Demonstrated successful completion of the SSH Credentialing process.
Nurse Practitioner license required. License, certifications, and classes will be monitored by the Medical Staff Services Department
