Job Description Summary
Under the direction of the Executive Director of the HCD, is responsible for supervising the development, implementation, and coordination of Quality Measures, Performance Improvement ,Education, and Reimbursement in the Home Care Division. ** Specialty teams include IV, Pedi, and Oncology**
Job Description
Knowledge of community health practice.
1 - Provides direct clinical supervision to assigned nursing staff.
a - Continues the clinical orientation of nursing staff after the didactic/preceptor orientation program is completed.
b - Has a general knowledge of each nurse’s caseload in terms of patient acuity and workload.( Case mix weight)
c - Assists each nurse in balancing the demands of patient acuity and required productivity.( Case management)
d - Has specific knowledge of all critical patient issues for the nursing team.
e - Provides direction and supervision to the nurses in resolving any patient issues.
f - Develops/ assures each clinician maintains skill in case management.
g - Coordinates the appropriate utilization of VNA and community services to prepare the patient for discharge throughout the Home Care episode.
h - Schedules /Recommends a Case Conference when necessary
2 - Provides field supervision of assigned nursing staff.
a - Joint visits regularly scheduled with all case managers
Makes routine home visits to observe/supervise staff providing care.
b - Makes specific home visits to assess critical medical, psychosocial, environmental, caregiver, etc., issues.
3 - Schedules and prioritizes team and or per visit nurses matching patient needs with the nursing
skill level, as evidenced by daily patient visit scheduling assignments.
a - Ensures adequate team staffing to coordinate care provided to patients in assigned locations.
b - Considers the workload of nurses when scheduling visits.
c - Serves as a triage nurse for patient, physician, and caregiver inquiries.
d - Resolves clinical issues.
e - Recommends and implements scheduling and staffing changes as appropriate.
4 - Demonstrates accountability for the quality of clinical services given by team nurses, as evidenced by correct completion of all admission and re–visit documentation and communication tools.
a - On admission, ensures that a Care Plan is prepared according to physician orders.
b - Ensures that the Care Plan is updated and individualized with each change in patient status and again every 60 days.
c - On admission, ensures that the Plan of Care is developed in collaboration with the patient/family/caregiver.
d - Ensures that a Plan of Care is developed, individualized and evolves, based on nursing diagnosis for each identified problem. Ensures that the intervention includes specific outcome criteria.
e - The Plan of Care will include, but is not limited to, self–care skills, disease process, medication, diet, activity, home safety management and emergency procedures.
f - Ensures that a teaching plan is developed, individualized and evolves.
g - Ensures that all appropriate visit–related data is communicated interdepartmentally/interdisciplinary.
h - Ensures that all required documentation is submitted within 24 hours of the visit.
5 - Demonstrates accountability that each nurse fulfills all responsibilities/duties required to address the unique nature of the patient being served, as evidenced by chart review and direct supervision.
a - Demonstrates and communicates knowledge of all applicable Organizational policies.
b - Demonstrates and communicates knowledge of Home Care Division Policies and Procedures.
c - Ensures that SSH and Home Care Division Policies and Procedures are implemented correctly and safely.
d - Ensures that the provider staff is able to safely and effectively provide care.
e - Ensures that all multidisciplinary services are provided on a timely basis.
f - Ensures that provider staff demonstrates knowledge of family violence, child/elder/disabled neglect and abuse so as to identify those at risk as evidenced by chart review and case conferencing.
6 - Reviews new patient referral information and identification of patient needs.
a - Ensures the scheduling of the patient visit to the most appropriate nurse. Ensures that the patient is visited in a timely manner.
b - Ensures that medical equipment and lab tests are obtained per physician order.
c - Ensures that patient emergency priority code is established on admission and updated during the episode of care
7 - Demonstrates accountability for optimal financial reimbursement of all team services.
a - Understands or seeks information on all insurance coverage criteria guidelines. Shares this expertise with staff.
b - Oversees the correct payor selection for each team patient.
c - Ensures that all clinical and reimbursement documentation is completed accurately.
d - Follows through with all unresolved financial issues.
e - Keeps the Director informed of any services not completely covered by insurance.
f - Demonstrates financial responsibility by monitoring identified specific reports regularly
8 - Promotes collaboration with other members of the healthcare provider team in order to ensure coordination of patient care.
a - Initiates and attends case conferences.
b - Ensures that all appropriate services are referred and involved in the case in a timely manner.
c - Recommends and assists the provider to access the appropriate area of service within the organization or community.
d - Ensures that all unplanned visits are appropriately responded to.
e - Ensures that all planned and unplanned HHA orientation visits are scheduled with the most appropriate professional provider.
f - Hand off communication is utilized, as appropriate, to patient issues.
9 - When the nurse–to–patient visit ratio exceeds accepted Division standards, and no further nursing coverage is available, will provide direct care to patients, as documented in the clinical record and on the daily log.
a - Completes patient care according to the standards and responsibilities of a community health nurse.
b - Submits an accurate and complete daily log.
10 - Participates in required UR/PIE clinical record reviews.
a - Ensures completion of required UR/QI clinical record reviews for all nurses supervised, as evidenced by completed UR Tools.
b - Participates in Specialized UR/QI clinical record review.
c - Monitors Team and individual team member compliance with documentation standards utilizing automated Management tools when available.
d - Participates in regularly scheduled meetings with PIE partners
11 - Advances knowledge of nursing practice by attendance at in-services and participation in seminars and courses to increase knowledge of community health nursing, as evidenced by attendance logs and CEU certificates
a - CPR re–certification is completed every two years.
b - Attends and participates in Management Team meetings and Division or Hospital subcommittees.
c - Attends in-service courses offered by the organization.
d - Attends in-service courses offered outside of the organization.
12 - Performs on–call manager duty on a rotating basis, as evidenced by completion of the on–call log.
a - Demonstrates sound triage and management of supervisory on–call issues.
b - Covers Physician representative calls as needed/assigned
c - Covers evening triage calls as needed/assigned
d - Cover each other’s teams as required
e - Accepts assignment of Director as needed
13 - Is responsible for Division’s operational excellence; assures delivery of quality services in accordance with the Home Care Division policies, procedures and professional standards.
a - Manages all team activities so that quality services are provided in an efficient and effective manner.
b - Services provided meet all applicable regulatory requirements.
c - Participates in an effective performance improvement program as evidenced by reports.
d - Maintains team productivity standards; monitors status and reports as requested.
e - Supervises department activities in a manner consistent with organization–wide programs and procedures.
f - Participates in the development of Divisional policies and procedures that reflect the organization’s standards.
14 - Manages human resources to assure quality services and promote positive employee relations.
a - Consistently and fairly implements human resources policies.
b - Maintains effective and appropriate staffing by monitoring employee turnover, overtime and absenteeism.
c - Employee performance appraisals are completed thoroughly and submitted on time.
d - Follows Human Resources policies; implements progressive corrective action when necessary.
e - Collaborates with Human Resources on the recruitment and selection of qualified employment candidates, following all policies, guidelines and applicable laws.
f - Develops performance goals/objectives with each staff member to promote maximum productivity, proficiency and professional growth, as evidenced by written goals on performance appraisals.

h - Participates in the Division’s clinical and managerial orientation process.
15 - Is responsible for the fiscal management of assigned nursing team; assures proper utilization of the Division’s financial resources.
a - Develops team budget within designated deadlines according to all budgetary policies and procedures.
b - Effectively utilizes resources within established budget; notifies Director of all variances anticipated, or actual.
c - Adheres to procedures regarding acquisition of, and payment for, goods and services, as evidenced by appropriate and timely processing requisitions and invoices.
d - Recommends way to reduce expenditures and/or enhance revenues without compromising quality of services.
e - Reviews fiscal reports as issued/available
f - Reviews Labor Distribution Report each payroll period.
g - Reviews HHG regularly
16 - Effectively communicates Divisional, organizational and industry information to staff.
a - Attends organization’s informational meetings, as evidenced by attendance sheets.
b - Gathers, interprets, and delivers information to staff in a timely fashion.
c - Assures staff understands information, as evidenced by staff compliance with and participation in organization programs, seminars, training and related activities.

17 - Embraces technological solutions to work processes and practices.
a - Demonstrates clinical proficiency and competency in the agency/program automated clinical record as evidenced by successful completion of education program.
b - Utilizes phone system and telecommunication services as instructed, this includes phone, email, paging systems, pagers and cell phones.
c - Oversees staff education related to the appropriate usage of assigned clinical equipment, e.g., IV pumps, PT machines.
d - Utilizes and is proficient on departmental or role specific software as assigned.
e - Demonstrates a base knowledge and utilization of the Microsoft suite of office products, e.g., word, excel, power point.
18 - Fosters a “Culture of Safety” through personal ownership and commitment to a safe environment.
a - Understands individual roles/responsibilities during emergency situations in patients’ home or at office, e.g. Emergency Preparedness Plan
b - Educates staff to and insures adherence to patient identification policies/practices
c - Educates staff and insures compliance with alarm testing on IV pumps and respirators to assure that they are set at appropriate settings
d - Use personal protective equipment appropriately
e - Adheres to respiratory etiquette guidelines
f - Handles hazardous waste according to Departmental protocol
g - Utilizes proper body mechanics when performing all aspects of job
