Performance Improvement Liaison

Job Summary:
Under the Manager of Performance Improvement and Education, participates in the various activities of performance and quality improvement, orientation, utilization/ reimbursement ,documentation, and coding oversight.  Initiates activities necessary to assure effective utilization of the Home Care Division's resources in order to maintain the highest quality of care achievable.
Equivalent to a Bachelor of Science Degree in Nursing from an NLN–approved school of nursing or Bachelor of Science Degree in Physical Therapy or Bachelor Degree in Occupational Therapy.  Three (3) to five (5) years of community health.  Physical Therapist/Occupational Therapist/Registered Nurse.  Knowledge of PI process and process evaluation techniques ( FMEA)  Demonstrated ability to interpret qualitative and quantitative data. Prepares cogent reports facilitating decision-making.   Facilitates innovation and is able to motivate groups towards solutions.  Creative thinking and problem-solving skills  Knowledge of Joint Commission, CMS, DPH and other regulatory standards and ability to interpret the standards.  Proficient with computerized software systems and technology.  Excellent verbal and written communication skills.  Ability to multi-task under stressful situations/environment.  Detailed oriented.  Solid Organizational skills.  Customer excellence skills
