Intake Clinical Coordinator

Responsible for initiating the registration process, determining the appropriateness for home care, creating positive relationships with outside referral sources, and responding to requests for information.

Essential Functions


1. Receives, creates positive relationship with referral sources and processes referral information for both VNA and Hospice patients that will initiate the admission process for home care services.

a. Collaborates with Referral sources to question and collect accurate information for safe and appropriate Home Care Referrals.

b. Completes intake screens for all referrals before time of admission

c. Coordinates visit date with clinical managers when appropriate and books laboratory test through appropriate lab when indicated on referral, following protocol .

d. Identifies at the time of referral patient need for specialty programs and initiates and maintains communication with managers around potentially complex admission.


2. Obtains insurance information on all referrals at the time of intake..


a. Collects reimbursement information and verifies client insurance status via web for MassHealth.

b. Maintains knowledge regarding scope of insurance contracts for home care services.

3. Independently assess patient data received from referral source for appropriateness for admission to home care or hospice based on Agency scope of service and Medicare Guidelines

4. Informs scope of Agency services to referral source at time of telephone contact.


a. Promotes positive Agency image with professional telephone manner at all times.

b. Acts as a Community Resource for both internal providers and external referral sources.

c. Facilitates referral process with ease of use for referral source

5. Assists referrer with alternative services when assessed patient needs fall outside of the VNA or Hospice Scope of service.

JOB REQUIREMENTS

Minimum Education - Preferred

Graduate of an accredited professional program such as nursing or rehabilitation services.

Minimum Work Experience

One to two (1-2) years clinical experience in community health preferred.  Experience in Home Health or ER highly preferred.

Required Licenses / Registrations  - Professional Clinical license such as one of the following:
Registered Nurse - Board of Registration in Nursing (Massachusetts)

Occupational Therapist - Board of Allied Health Professions (Massachusetts)

Physical Therapist - Board of Allied Health Professions (Massachusetts)

Required additional Knowledge and Abilities

Knowledge of electronic documentation systems, typing and telephone skills required. Home care coding and intake processes preferred.  Must be able to take and act on MD verbal orders.
