Hospice Aide

Job Description Summary

A trained, certified, para–professional, who is prepared to assist professional clinical providers (RN or Therapist) in the care of the patient in the home and provides direct home and personal care services for the sick and/or disabled according to the plan of care. The duties of the Home Health Aide/HCA III are planned and assigned by the professional clinical provider to meet the patient and caregiver's needs. The Home Health Aide/HCA III is not a substitute for the professional clinical provider, but he/she may perform patient care under the direct supervision of the professional clinical provider.

Job Description

Responsible for direct patient care which is determined, observed, supervised and documented on plan of care by a professional clinical provider. 

Must be able to drive/or use public transportation to patients’ homes

  • Helps patient with bathing, dressing and care of mouth, skin, hair and fingernails.
  • Assists with ambulation, transfers and follow-through with use of special equipment.
  • Assists with and encourages patient's participation in prescribed exercises  
  • Encourages independence and self-care.
  • Helps patient to bathroom, or in using commode, bedpan or urinal.
  • Prepares meals according to prescribed diet, while insuring adequate/appropriate fluid intake.  Assists patients unable to feed themselves.
  • Uses communication skills appropriately to provide emotional support.
  • Changes bed linen on an occupied/unoccupied bed.

Performs delegated tasks as authorized by the professional clinical provider depending upon situation and experience/competence. When instructed, reminds and assists patient to take pre–poured oral medications that have been prescribed by the patient's physician.

Performs routine household tasks as related for a safe, healthy and comfortable environment for the patient, as instructed and documented on plan of care by the professional clinical provider.

  • Maintains a neat, clean, safe environment for patient by performing household tasks, as instructed on plan of care.
  • Prepares and serves meals according to dietary instruction.
  • Assists with patient’s laundry as instructed on plan of care.

Documents patient status and care given, as reflected on required records.

Works as part of the Home Health Care Team participating in and identifying ways to improve the quality of care being provided to the patient. Communicates effectively with Scheduling Coordinator to assure that all schedule changes go through office. Recognizes and reports changes in the patient's condition including changes in comfort/pain level or family situation to the professional clinical provider/manager in a timely manner.


Minimum Education – High School or equivalent preferred

Minimum Work Experience - Prior experience is preferred

Required Certifications -

CNA or HHA or HHA Eligible - Certified Nursing Assistant or Home Health Aide or Home Health Aide Eligible
